1.  Fill out the order form below and press "submit," which will generate an order confirmation page and will reserve your CD.  Print the confirmation page for your records.

2.  Send check or money order payment ($12.50 per CD) to the address shown on the order confirmation page. (Please make money orders or checks payable to "T.A. Burrows" and please do not send cash.)

3.  Once the payment has been received, we will immediately ship your CD.
